Registration Instructions


To receive the Custom Fit discounted price:
  1. If your company has already been qualified by Custom Fit, you must work through your designated company contact person. That person should fill out a Registration Form / Training Agreement* and return it to the MATC.
     
  2. If you don’t know if your company is qualified, please contact us.
       
  3. To qualify your company for Custom Fit funding, click here.

* Click here for a Registration Form / Training Agreement.
You may wish to save the Training Agreement document on your computer for future classes.  If you do, you won't need to fill out the company information section more than once.
